Please use our HIPAA-compliant form to securely upload medical records, test results, x-ray images and any other files that you would like to share with our practice
If you would like to upload multiple files, once you get a confirmation that the file was successfully uploaded after pressing the ‘Upload File’ button, you can choose another file and click ‘Upload File’ button again (without having to re-enter the patient info), and you can repeat it for each of your files as needed.
You enter comments, orders and directions in the Notes section.
Alternatively, you may consider combining them into one compressed file (e.g., a Zip file) prior to uploading.
