As a new patient, we want you to feel welcome. In order for us to serve you in the best possible way, we will review your medical and dental history, questions, and concerns together
Please fill out our online patient information form to the best of your ability. If you have any questions, please contact us and we will be happy to assist you
If you are an existing patient, in order to make sure that we have all your information up-to-date, please take a moment to update your contact information and communication preferences (e.g., to opt-in for receiving invoices via email)

You can securely upload your health records that you would like to share with us
You can also share this link https://demo1.entrespace.com/patients/upload with your doctors and healthcare providers so that they can directly upload records they would like to share with our practice on your behalf
If you would like to upload multiple files, once you get a confirmation that the file was successfully uploaded after pressing the 'Securely Upload' button, you can choose another file and click the 'Upload File' button again without having to re-enter the patient information. You can repeat that for each of your files as needed
Alternatively, you may consider combining them into one compressed file (e.g., a Zip file) prior to uploading
